CDI PREREQUISTE CLEARANCE REQUEST FORM

As of July 1, 2012 all students enrolling in CDI classes must have taken the designated prerequisites for the course(s) they wish to enroll in. Students who have not taken the designated De Anza CDI classes may submit a Pre-Requisite Clearance Form which details which course they wish to enroll in and what they have completed previously to meet the prerequisite requirements. The form along with supporting documents must be submitted to the CDI Department Coordinator at lamitgary@fhda.edu
