You must provide the following documents.
(A) Catalog description from prior college for course(s) that you want considered for prerequisite clearance AND
(B) College transcript with final grades for courses from prior colleges that you want considered for prerequisite clearance.
For your transcript, include either an Official Transcript in a sealed envelope (if you plan to attend De Anza for two quarters or more) OR an Unofficial Transcript (if you plan to attend De Anza for only one quarter).
Foothill students do not need to attach a Foothill transcript or course description.
Submit your request at least two weeks before your registration date.
You will receive an e-mail regarding your request within 3-5 working days after your paperwork is received. Please update your personal e-mail information in MyPortal and check your e-mail often.
